See the listing of incidents below. Employers must report worker fatalities to OSHA within eight hours. OSHA investigates all work-related fatalities in all covered workplaces. Federal and state OSHA agencies have up to six months to complete an investigation and determine whether citations will be issued.

05/10/2017 Oelwein Worker died after falling and striking head on floor.
05/01/2017 Council Bluffs Two Deputies fatally shot by inmate.
04/22/2017 Keokuk Worker drowned when cart fell into dam.
04/13/2017 Marshalltown Worker died after being struck by piping plug checking air tank for leaks.
04/03/2017 Davenport Worker died in vehicle collision.
03/27/2017 Belmond Worker died in fall inside grain bin.
03/10/2017 Kalona Worker died after being struck by lifting device.
03/07/2017 Des Moines Worker died after being struck by truck.
02/27/2017 Yarmouth Worker died in fall from back of truck.
